How the Advance HMO Approval Request Works
The online form asks for only the essentials. In less than three minutes, you can complete and submit your request:
- Personal Information – Name, contact details, and valid ID upload.
- Medical Request Details – Reason for visit (consultation, lab, procedure, etc.).
- HMO & Insurance Information – Provider name, member ID, and a photo of your health card.
Once submitted, our coordination team works with your HMO provider on your behalf. You will receive an approval confirmation via the email address or mobile number you provided. That’s it – you are cleared for your visit.
